Dream Pursue Grow Support is committed to protecting the privacy, dignity and confidentiality of participants, families, carers, workers and referrers.

This page explains, in plain language, how personal information may be collected, used, stored, shared and managed as part of service delivery, enquiries, referrals and ongoing support.

It is based on Dream Pursue Grow Support’s internal Privacy and Confidentiality Policy and Information Management Policy and Process.

What information we collect

Dream Pursue Grow Support only collects information that is reasonably necessary to provide supports, respond to enquiries and referrals, manage services, maintain records and meet legal, contractual and professional obligations.

  • Information needed to understand support needs, goals, health or service requirements
  • Information provided by participants, authorised representatives, families, carers or referrers where appropriate

How information is used

Personal information may be used to assess needs, plan, deliver and review supports, communicate with participants and authorised representatives, coordinate with relevant providers, and meet billing, risk, quality and compliance requirements.

Consent and sharing

Dream Pursue Grow Support seeks consent before collecting, using or sharing personal information unless an exception applies under law or disclosure is necessary to prevent or lessen a serious threat to life, health or safety.

  • Information is shared only with people or services who need it for the participant’s support, or where legally required
  • Participants are informed about why information is being collected and how it may be used

Storage and security

Information is stored securely using protected electronic systems and, where relevant, locked paper records. Access is limited to authorised personnel, and reasonable safeguards are used to protect information from loss, misuse, unauthorised access, modification or disclosure.

Access and correction

Participants, or their authorised representatives, may request access to personal information held about them and may ask for corrections if information is inaccurate, incomplete or out of date.

Complaints and concerns

Privacy and confidentiality concerns can be raised directly with Dream Pursue Grow Support. Concerns and complaints will be managed seriously, promptly and fairly, and information can be provided about external complaint options where relevant.
